The effects of inhaled CO in the prevention of I/R injury by Dr Atsunori Nakao
In 2003, Dr Atsunori Nakao of the University of Pittsburgh Transplantation Institute received a GEMI Fund grant to study the effects of inhaled CO in the prevention of I/R injury in transplantation. This is a brief summary of his findings.
I/R (ischaemia/reperfusion) injury refers to the inflammatory response occurring when blood flow to the cells is stopped (in this case during transplantation), then the area is reperfused with blood and inflammation occurs. This results in ischaemia (or injury) which can limit perfusion followed by further reperfusion-induced inflammation.
Dr Nakao and his team experimented with transplantation of rat organs, including liver, kidney and intestines. As well as considering the impact of CO therapy, they also looked at the possible mechanisms at work.
The research considered the effect of CO therapy on the organ and on the recipient, as well as the optimal dosage.
The results were very positive and suggest that CO could be a useful adjunct in protecting organ grafts from various I/R injuries associated with transplantation. These investigations into experimental transplantation with rat models have revealed that CO provides a significant level of tissue protection during I/R injury, during alloimmune responses (when the body reacts to the transplantation of a ‘foreign’ organ) and if the transplanted organ begins to deteriorate.
A key challenge in the future will be to attempt to translate these exciting discoveries into new therapeutic modalities within the clinical transplantation setting. The unique clinical circumstances of transplantation in fact provide multiple opportunities to apply CO therapeutically; it can be administered to the donor, to the excised grafts (such as organs) and to the transplanted recipients.
